Key features with the COSE Benefit Plan
Member Choice
Offer alternative coverage options with the same deductible at different price points and save up to 15% in funding rates. This gives employees choice in plan design selections while not having to choose between deductibles.
Multi-policy Bundling Credit
Bundle dental, indemnity, or life plans with one of our self- or level-funded small group health plans and receive a one-time invoice credit. Savings vary based on the number of enrolled employees and the products you add.
Network Strength
Access healthcare anytime, anywhere through our narrow to broad network options with more than 12,000 primary care doctors, 46,000 specialists and 19,000 locations across Ohio, plus national network access through Cigna when traveling out-of-state.
How to Qualify
All qualifying small businesses must enroll and maintain good standing with the Toledo Regional Chamber of Commerce. If you are not in the designated chamber region, you can join through another affiliate chamber or COSE.
