Updates and Reminders

1094/1095

Starting Feb. 24, 2023, members may access their 1095-B forms directly from My Health Plan (MHP). Due to recent legal requirements, we are no longer required to send 1095-B forms by mail. Therefore, we will only mail forms to members who call Customer Care and request a hard copy.

1094-B forms are filed with the IRS on behalf on the employer.

Sole Proprietor/Group of One Affidavit

The Group of One Affidavit may not be required in all instances. Use these guidelines as a starting point, if there are questions underwriting may follow-up.

Examples of when Group of One Affidavit may not be required:

  • There are W2 wages for an employee other than the owner (ie: part time employees)
  • There is a quarterly wage and tax report available
  • A husband and wife only business where one of them shows up on quarterly report as W2 employee
  • Tax forms provide evidence of full-time business revenue

Paperless Billing

Encourage your groups to enroll in paperless billing through EmployerLink. Groups that sign-up for EFT withdrawal also avoid the $25 monthly admin fee. EFT is withdrawn the first week of the month. Groups can view their invoice on EmployerLink and can sign-up for email notification

Enrollment Changes

Once enrolled, enrollment changes should be processed through EmployerLink or sent to MemApps@medmutual.com (not sent through FormFire or EasyApps).

Ordering ID Cards for Your Customers

ID cards can be ordered online through MyBrokerLink, My Health Plan or viewed on the Medical Mutual mobile app.
